Between Q3 2025 and Q1 2026, we interviewed 34 businesses who had terminated an AI agency engagement within 12 months of starting. We recruited through LinkedIn, referrals from solicitors who handled contract disputes, and direct outreach to companies who'd posted public complaints. Participants received no payment. The interviews averaged 47 minutes and were conducted under NDA — all data is aggregated and anonymised.

Failure Pattern #1: Misaligned Success Metrics

In 19 of 34 cases, the client and agency measured success differently. The agency reported 'tasks automated'; the client expected 'cost reduced.' These are not the same thing. An agency can automate 10 tasks and save zero money if those tasks were already low-cost or if the implementation requires expensive oversight. Contracts must define success in business terms: £ saved, revenue generated, or hours recovered — not technical metrics.

Failure Pattern #2: Client Infrastructure Not Ready

14 of 34 cases involved infrastructure the agency didn't audit before signing: legacy systems with no API access, data scattered across systems with no integration path, or workflows that lived entirely in human heads. The agency discovered these realities post-contract and faced an impossible choice: do unscoped work for free or disappoint the client. Both paths ended the relationship.

Failure Pattern #3: No Milestone Gates

In 27 of 34 failed engagements, the contract defined deliverables at the end of the engagement, not at 30, 60, and 90-day checkpoints. This allowed problems to compound invisibly. By the time the client realised the engagement was off-track, months had passed and trust was gone. Milestone gates create forcing functions: if a gate isn't met, both parties stop and reassess before more time and money are invested.
